A picture ID and insurance card are required for services from BCBSM, as well as HealthPlus, Blue Care Network and Medicaid. These are the only insurances that are being accepted by the health department at this time. A photo ID is also required to receive an additional copy of a shot record. Before coming to the clinic for services, clients should first call their insurance companies for verification that they do have coverage for the needed vaccines of the insured recipient. IF payment is denied to the health department, they will then bill the parents and/or the client that received, so it is important to know the correct information before coming to the clinic. Individuals covered by companies other than those listed above should also call to verify coverage and if so, whether the company would reimburse them if they had a receipt with the billing codes. The health department is unable to accept the auto groups listed below: Daimler Chrysler, group 82100 to 82900; Delphi, group 72100 to 72524; Ford, group 87240 to 8727 and 87950 to 87973; General Motors, group 83100 to 83540; and Visteon, group 73111 to 73200. These auto group numbers will need to return to their physician's office or pay full price for the vaccines that are needed. Further information from the health department will be on their website and news releases to keep updated information available. In the meantime, for more information, contact (989) 758-3840.
